Preparer Complaint Form

Did Your Tax Preparer Sign? 

Paid tax preparers are required by law to sign federal and state tax returns. They are also required to include an IRS Preparer Tax Identification Number (PTIN) on federal tax returns they prepare for a fee. Please fill out the complaint form if your tax preparer did not sign your tax return by name (typed or handwritten), signed it "self prepared" or signed it with a business label

Helpful Complaints include:

  • Date of Service
  • Copy of the tax preparer's business card
  • Amount paid for services
  • Tax forms filed by tax preparer
  • Copy of filed tax returns

Please provide as much information about the tax preparer you are reporting. If additional information is needed, you will be contacted. If not enough information is received and you do not provide the proper contact information, your case could be closed. 

All information you provide (including name and contact information) is kept confidential due to privacy disclosure laws. Reports are sent directly to the California Franchise Tax Board Tax Preparer Enforcement Team.
